https://gxygcg.ejy365.com/admin/app-api/pm/purchase/5.1.2具有房屋建筑工程乙级及以上工程监理资质证书；
https://gxygcg.ejy365.com/admin/app-api/pm/purchase/5.1.3具有机电安装工程乙级及以上工程监理资质证书；
https://gxygcg.ejy365.com/admin/app-api/pm/purchase/5.1.4认为有必要提供的其他声明及文件资料（提交资质为工程监理综合资质的可不提供上述专业资质）。

https://gxygcg.ejy365.com/admin/app-api/pm/purchase/②被信用中国网(www.creditchina.gov.cn )或中国政府采购网(www.ccgp.gov.cn )等渠道列入失信被执行人或重大税收违法案件当事人名单，或被列入政府采购严重违法失信行为记录名单（以评审阶段通过上述网站查询的结果为准，对于成交人，如在签约之前在上述网站被列入失信被执行人或重大税收违法案件当事人名单的，采购人有权取消其成交资格）。

https://gxygcg.ejy365.com/admin/app-api/pm/purchase/7.2现场领取：潜在的合格报价人可于采购文件获取时间内到贵州金久水泥有限公司综合办公室一楼后勤主管办公室领取文件。领取文件时需提交加盖单位公章的采购项目确认表（附件1）、营业执照复印件。
7.3线上领取：潜在的报价人可于采购文件获取时间内通过电子邮件或微信形式将加盖单位公章的确认表扫描件（附件1）、营业执照原件扫描发送至794050524@qq.com邮箱。

https://gxygcg.ejy365.com/admin/app-api/pm/purchase/8.1 报价文件递交截止时间为2025年4月26日下午17：00 (北京时间)，地点为贵州金久水泥有限公司（贵阳市修文县久长街道金桥村贵州金久水泥有限公司综合办公室一楼后勤主管办公室，收件人：张先生：注册后查看。）
8.2递交方式：现场递交或快递邮寄方式递交报价文件，送达时间按快递送达采购人时间为准，因邮寄方式导致报价文件逾期送达或密封破损，导致采购人拒绝接收的，由报价人自行负责。
